He does it out of a violent, damaged, unhealthily codependent need to have his brother existing, living, even if it's a depraved and self-destructive existence because as far as Stefan is concerned, the world isn't right without Damon.
Even when their relationship was at it's worst, even when Damon killed his best friend, Stefan couldn't bring himself to cross that line and end his brother's life. Damon, at the end of the day, is all he has. If Stefan loses that, eternity would seem just a little bit emptier.
Still, he's used to Damon wrenching away from him, and he watches the tension in his shoulders, trying to get a read on what exactly is happening inside Damon's head, even though he never really could.
